SafeCare BC Survey Request About Flu Vaccination

We are requesting your participation in a brief, 15-minute survey so that we can learn from you and improve SafeCare BC’s vaccine resources.

We are running an online survey to understand your experiences with the flu vaccine. In this 26 question survey, we will ask you how you feel about the flu vaccine, whether you have been vaccinated, your motivations or hesitations to get vaccinated and any concerns you may have. We will also ask you about where you get your flu vaccine information and how this information could be improved.

You will only be able to take the survey once on a single device. Therefore, if you are accessing the survey from a work computer, it will not be possible for multiple people to take the survey on the same device.

We will hold a prize draw for five $50 visa gift cards. If you would like to be entered in the prize draw, you will be asked to provide your email at the end of the survey. This email will be collected through a separate survey link to maintain your anonymity.

At the end of this survey, you will also have the opportunity to indicate whether or not you are interested in participating in a follow-up interview about your experiences. If you choose to include your contact information, we will anonymize the information you share with us by removing your email from your responses to maintain your confidentiality. If you choose not to include your contact information, this survey will be anonymous.
